I hereby waive, release, and discharge any and all claims for damages for personal injury, death, or property damage which I may have, or which may hereafter accrue to me, as a result of participation in any activities or events organized by Out Hustle Athletics LLC & Coach Miele Sports & Games, including but not limited to games, sports, arts and crafts, STEM activities, or other event-related activities. I fully understand that participation in these activities may involve physical exertion and inherent risks, including the possibility of injury. As a parent/guardian, I acknowledge these risks and consent to allowing my child to participate in the activities provided by Out Hustle Athletics LLC & Coach Miele Sports & Games. I confirm that I have insurance and agree to hold the coaches, staff, volunteers, and organization (Out Hustle Athletics LLC & Coach Miele Sports & Games), as well as any facility where events are held, harmless in the event of any injury or incident. This release is intended to discharge in advance Out Hustle Athletics LLC & Coach Miele Sports & Games, its coaches, owners, volunteers, and associated facilities from any and all liability arising out of or connected to my or my child’s participation in event-related activities. I acknowledge that these activities involve elements of risk and danger, and I voluntarily assume those risks. I agree to indemnify and hold Out Hustle Athletics LLC & Coach Miele Sports & Games, as well as all persons and entities associated with the organization, free and harmless from any loss, liability, damage, cost, or expense arising from any injury, death, or property damage sustained during participation in these activities. I also grant Out Hustle Athletics LLC & Coach Miele Sports & Games, its coaches, staff, and volunteers the right to copyright, publish, reproduce, or otherwise use my child’s name, voice, likeness, and photographs or audiovisual recordings of my child for promotional materials, including but not limited to advertising, websites, publications, or brochures. I hereby consent to my child’s participation in the activities organized by Out Hustle Athletics LLC & Coach Miele Sports & Games and execute this agreement, release, and waiver on their behalf. I affirm that my child is physically able to participate in the activities. I agree to indemnify and hold harmless all persons and entities mentioned above from any loss, liability, damage, cost, or expense incurred as a result of any injury, death, or property damage sustained by my child during participation. I have carefully read and fully understand the contents of this agreement, waiver, and release.
