Fall Basketball Youth Teams

Program Outline:
Boy's Teams: 6th/5th Grade (Combined Team) 4th Grade/3rd Grade (Combined)

Practice Location: The Harvey School in Katonah, NY

Insurance: Out Hustle Athletics LLC is fully insured to protect our athletes and coaches.

Tryouts: Wednesday, August 31st from 7:30-8:45 PM

Practices: Practice and skill development is important for continued development. Every team practices once a week. Practice location will be at the Harvey School in Katonah, NY. All practices are 60 minutes in duration.

Practice Dates:
September: 7th, 14th, 21st 28th *Only 9/28, practice will be combined due to time change based on an event at Harvey*
October 6th, 12th , 19th 6:30 PM – 8:30 PM (October 6th is a Thursday, the school is not available on the 5th due to Yom Kippur)

4th Grade: 6:30 PM to 7:30 PM
5th Grade 7:30 PM to 8:30 PM
*Wednesday, September 28th practice will be combined from 7:15 PM – 8:30 PM.

League: Team's will play 4 games plus playoffs from August-October. Games will be at the Hardscrabble Club in Brewster, NY.

Out Hustle Athletics LLC has no affiliation with any High School basketball programs in Westchester County, NY. This is an independent company and by playing for or within the organization does not guarantee you a roster spot on any High School team in which your child attends or plays for.

I hereby waive, release and discharge any and all claims for damages for personal injury, death, or property damage which I may have, or which hereafter accrue to me, as a result of participation in basketball practices and/or basketball competitions in all its forms. I fully understand that basketball is a contact sport, it is a physical sport, players of all skill levels may be injured practicing or playing the sport, as a parent/guardian I understand the risk and consent to letting my child play basketball for Out Hustle Athletics LLC, I also have insurance and agree to hold coaches and organization (Out Hustle Athletics LLC) and any facility we practice in harmless in case of any injury. The release is intended to discharge in advance, the AAU team (Out Hustle Athletics LLC, its coaches, owners or volunteers and facility from any and all liability arising out of or in any way connected with my participation in said activity. It is understood that the activity involves an element of risk and danger of accidents and knowing those risks, I assume those risks, and I agree to indemnify and to hold the Out Hustle Athletics LLC, all persons and entities associated with the Out Hustle Athletics free and harmless from any loss, liability, damage, cost or expense, in which they may incur as a result of my death, injury or property of damage that I may sustain while participating in the above activity. I also give the AAU team Out Hustle Athletics LLC, it’s coaches or volunteers) the right to copyright and/or publish, reproduce, or otherwise use my child’s name, voice, and likeness and/or photographs, and audiovisual recordings that include my child for instruction, advertising, program website, publications or brochures, or any other lawful purpose whatsoever. Parental Consent: (to be completed and signed by parent/guardian if applicant is under 18 years of age). I hereby consent that my child, may participate in the above activity, and hereby execute the above agreement, release and waiver on his behalf. I state that my child is physically able to participate in said activity. I hereby agree to indemnify and hold the persons and entities mentioned above free and harmless from any loss, liability, damage, cost, or expense, which may arise or may be incurred as a result of death, injury, or property damage that said minor may sustain while participating in said activity. I have carefully read this agreement, waiver and release and fully understand its content. I understand that Out Hustle Athletics LLC has no affiliation with any High School basketball programs in Westchester County, NY. This is an independent company and by playing for or within the organization does not guarantee you a roster spot on any High School or Middle School team in which your child attends or plays for. WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT In consideration of being allowed to participate on behalf of (Out Hustle Athletics LCC) athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that: Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Out Hustle Athletics LLC) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
