Spring Skill Session Program

Are you looking for a basketball skills program? Out Hustle Athletics is excited to provide a Spring Skill Session Program. All athletes will participate in basketball skill development specifically designed to improve their individual skills. Curriculum and daily lessons will be planned to accommodate social distancing guidelines. However, parents should be aware limited frequency of closer spacing is possible at times but will try to be avoided at all times. No LIVE GAMES will be played. All athletes must provide their own basketball and water bottle for safety purposes.

I hereby waive, release and discharge any and all claims for damages for personal injury, death, or property damage which I may have, or which hereafter accrue to me, as a result of participation in basketball practices/games or camps and/or basketball competitions in all its forms. I fully understand that basketball is a contact sport, it is a physical sport, players of all skill levels may be injured practicing or playing the sport, as a parent/guardian I understand the risk and consent to letting my child play basketball for Out Hustle Athletics LLC, I also have insurance and agree to hold coaches and organization (Out Hustle Athletics LLC) and any facility we practice in harmless in case of any injury. The release is intended to discharge in advance the AAU team (Out Hustle Athletics LLC, its coaches, owners or volunteers and facility from any and all liability arising out of or in any way connected with my participation in said activity. It is understood that the activity involves an element of risk and danger of accidents and knowing those risks, I assume those risks, and I agree to indemnify and to hold the Out Hustle Athletics LLC, all persons and entities associated with the Out Hustle Athletics free and harmless from any loss, liability, damage, cost or expense, in which they may incur as a result of my death, injury or property of damage that I may sustain while participating in the above activity. I also give the AAU team (Out Hustle Athletics LLC, it’s coaches or volunteers) the right to copyright and/or publish, reproduce, or otherwise use my child’s name, voice, and likeness and/or photographs, and audiovisual recordings that include my child for instruction, advertising, program website, publications or brochures, or any other lawful purpose whatsoever. Parental Consent: (to be completed and signed by parent/guardian if applicant is under 18 years of age). I hereby consent that my child may participate in the above activity, and hereby execute the above agreement, release and waiver on his behalf. I state that my child is physically able to participate in said activity. I hereby agree to indemnify and hold the persons and entities mentioned above free and harmless from any loss, liability, damage, cost, or expense which may arise or may be incurred as a result of death, injury, or property damage that said minor may sustain while participating in said activity. I have carefully read this agreement, waiver and release and fully understand its content. I understand that Out Hustle Athletics LLC has no affiliation with any High School basketball programs in Westchester County, NY. This is an independent company and by playing for or within the organization does not guarantee you a roster spot on any High School or Middle School team in which your child attends or plays for. I am aware of the risks of contracting or spreading Covid-19 while participating in any Out Hustle Athletics LLC program or event. I agree to and will follow all guidelines for personal hygiene, personal safety and public safety as recommended by Out Hustle Athletics LLC and NYS. This may include, but is not limited to, maintaining social distance; limiting the number of people attending and washing my hands prior to and following each session or activity; use of hand sanitizer upon request; wiping down surfaces with disinfecting wipes and/or wearing a mask and/or gloves. I agree to stay home and/or cancel session with Out Hustle Athletics LLC should I have personally exhibited or have been in contact with someone who has presented with illness within the previous 24 hours to 2 weeks, including; cough, sneezing, fever, chest congestion or additional signs of potential spread of any virus or bacteria/disease. I agree to follow these and hold harmless all individuals associated with Out Hustle Athletics LLC. BY CLICKING YES BELOW, I CONFIRM THAT I HAVE READ AND UNDERSTAND THIS RELEASE.
